If you have been involved in an automobile accident you must complete and forward to DMV within 10 days a DMV 104-A form, if:

1) Any person is killed or injured. ( No matter how slight the injury );
2) Any involved vehicle sustained $1000.00 or more in damage.
Failure to notify DMV may result in the suspension of your driving privileges.
If you received a Uniform Traffic Ticket for an Equipment Violation (i.e., Inadequate Headlamp/Taillamp or similar) you can have the faulty item repaired and inspected by a Police Officer or a NYS Certified Repair Shop within 24 Hours after the Ticket was issued.  Print this form, complete it, have the Inspecting Officer / Repairman complete the bottom of the form and Mail it to the Court listed on the ticket along with the ticket.
